When a goal includes the specific tasks a student should complete, those tasks should actually be on the data collection sheet. If it's math facts (John will correctly answer math facts for addition with sums from 0 to 10) the math facts should either be checked off, or a place should be created on the data sheet where you can write the facts that John got incorrect, in order to drive instruction. Jun 11, 2012 · What I plan on doing is giving each student a “Me Binder,” that contains all of the Goal Tracking Sheets I have created for them. Behind each goal sheet, will be actual activities and assessments for the student to complete that will provide me with the data I need to report on.

The place to begin is probably with a sample student tracking sheet that contains student-friendly learning targets. My students use this to track formative assessment and self-assessment data. The goal is for them to be cognizant of the learning intention and their progression toward mastery of it. www2.education.uiowa.edu
